Hello, wondering if someone might know the answer to this question...I have a provider that bills the Q3014 telehealth site fee with a consult code and I see a lot of denials for this code. I have noticed when we bill the place of service as "TH", the consult will pay but the Q3014 denies as included or non-covered. But if we bill the place of service as "O", both codes will pay. Any thoughts?

Q3014 is the fee for the telehealth originating site facility fee which is where the patient is located and this code is intended to reimburse the facility/clinic for their expenses related to the patient's use of their supplies and staff. There should not be any professional fees billed by the originating site because the treating provider is at the distant site and bills separately for their professional fees. So, if your provider is providing a consult that means your provider is at the distant site; therefore, your provider cannot/should not be reimbursed for the originating site fee represented by Q3014.

Where is the patient located when your provider is performing their consult service? That location could be eligible to bill Q3014 if it one of the sites authorized by law, which are: Physician or practitioner's office; Hospital; Critical access hospital; Rural health clinic; Federally qualified health center; Community mental health center; Skilled nursing facility; Hospital-based or CAH-based renal dialysis center. If the patient is in any other location than one of these locations Q3014 is not billable by the originating site.

If our NP does a televisit from our physician's office, and patient is located at home, can NP bill this code Q3014?

No, your NP cannot bill for Q3014 because it is for the originating site, which is the where the patient is located during the telehealth visit. In this case the patient was in their home, so that is the originating site, your NP is at the distant site, which is where the rendering provider is located during the telehealth visit. Your clinic did not incur any expenses for the patient's use of their home as the originating site for the telehealth visit, so there isn't anything you should be reimbursed for in this case.

If the provider is working remotely, and the patient comes to the office/clinic for a video/audio telehealth appointment, can Q3014 be used? POS-02 99213-95 with Q3014.

Hi there, you can't report Q3014 in that scenario. The fact that the provider isn't in the office for the encounter doesn't make the office an originating site. If you're billing Medicare you don't need modifier 95, but you should report POS 02.
