Please join the Documentation Advisory Committee as we dive into learning how to read operative reports. What are the basics you need to abstract in an operative report to code accurately. How do the coding guidelines fit in? New coders and experienced coders alike can benefit as we review cases from orthopedics, gastroenterology and cardiology.

Learning Objectives/Agenda:

  • What is the key information to be abstracted in an operative report?
  • What to do when there are terms you do not understand?
  • How do the surgical guidelines for CPT impact code selection?
  • We will review aspects of coding specific to orthopedic, gastroenterology and cardiovascular.
  • Review the associated and potential surgical modifiers common to these procedures.

Why is this topic timely/important?

This will be a great session for new coders who need assistance with reading and interpreting operative reports. With the guideline changes for hernias and the OIG reviewing of facet injections, both topics will be covered as we look at operative reports.

Who would benefit from this topic?

New coders for sure. Those who are entering new specialties. Coders who may have to fill in for others in areas they do not usually code. Those who are looking for more practice in dissecting operative reports. Additionally, this will be a good session for those in supervisory and administrative roles as we review the current OIG focus on some of these topics.

Melissa Kirshner, MPH, CPC, CPCO, CDEO, CPMA CRC, CEMC, CFPC, COBGC, Approved Instructor, AAPC Fellow, is executive director of a physician organization in Southeast Michigan. She has more than 30 years of healthcare experience in billing, coding, compliance, education, and software development. As a certified PMCC instructor, she teaches billing and coding classes to prepare students for the CPC® exam. Kirshner is a founding member of the Novi, Mich., local chapter, and serves as treasurer.

Julie Davis, COC, CPC, CPCO, CDEO, CPMA, CRC, CEMC, CFPC, Approved Instructor has 20+ years of experience in coding, auditing, compliance, documentation improvement, consulting, and teaching. She has worked with large multi-specialty groups, small billing companies, consulting companies and educational institutions. She is currently a member of the AAPC Documentation Advisory Committee, and teaching in the VILT courses for AAPC in addition to her role as a Risk Adjustment Manager for an IPA in Denver, Colorado. She has held officer positions in chapters in both California and Colorado. Her focus is to pay forward all the help and support that she has received in her coding career.

Elizabeth Herbert, RHIA, CPC, CPMA, CRC, CCC, Approved Instructor is an Epic application analyst for Baptist Healthcare System in KY & IN. She has more than 20 years in the healthcare field with billing, coding, education, pharmacy and clinical experience. She serves as member development officer for the AAPC New Albany Indiana local chapter and as a subject matter expert on the AAPC Documentation Advisory Committee.

Chelsea Kemp

Ms. Kemp is currently an Outpatient Coding Educator/Auditor at Yale New Haven Health, Subject Matter Expert at AAPC, and Adjunct Instructor at Central Arizona College. Chelsea has expertise in emergency department and gastroenterology coding, AZ state tape edits, NCCI, medical necessity and other billing/coding edits. She also is experienced in observation, OB Triage, outpatient ancillary and ambulatory surgery coding, as well as training and educating new coders in outpatient facility coding.

Kelly Shew, RHIA, CPC, CPCO, CDEO, CPB, CPMA, CPPM, CRC, CEMC, CGIC, CPEDC, Approved Instructor made the switch to healthcare about 12 years ago. Since then, she’s spent some time billing, coding, and auditing, but her happiest places are in education and risk adjustment. She currently serves as the Education Officer for the Ann Arbor AAPC chapter and is also a member of the AAPC Documentation Advisory Committee.
