Is that enough provider documentation for a telehealth visit with a 6 year old patient? Does that provide enough information to say guardian consents to visit? I hate when providers delete templates and add their own wording!!!

I would question why the patient and Mom are in online/on the phone with the physician before consent to the service was obtained. Surely, consent can be obtained at the time of scheduling, as part of the log-in to the system, or even as a one-time or once per year agreement, as allowed under state or payer regulations. Were consent obtained prior to the physician's work beginning, "Mom with patient" provides the necessary information of who was present for the encounter.
