Question-for anyone working pathology coding-since there are no finds in the lymph nodes- is the code to use I89.9 or should the cancer code be used?
I think the I89.9The ICD-10 code "I89.9" refers to a "Noninfective disorder of lymphatic vessels and lymph nodes, unspecified" meaning a general diagnosis for any non-infectious issue related to the lymphatic system without a more specific detail provided. - I was told this is not correct and only the Cancer code should be used- there is no mention of cancer only neg for Metastatic
there is no findings

A. LYMPH NODE, EXTERNAL JUGULAR, EXCISION:
--ONE LYMPH NODE, NEGATIVE FOR METASTATIC MELANOMA (0/1), SEE COMMENT

COMMENT: Routine and immunostained sections including Melan-A and SOX10 were reviewed. Isolated small bland-appearing Melan-A and SOX10-positive melanocytes are focally noted in the capsule. While of limited cellularity, this finding is compatible with a nodal nevus.

B. LYMPH NODE, LEVEL 2, EXCISION:
--ONE LYMPH NODE, NEGATIVE FOR METASTATIC MELANOMA (0/1)

COMMENT: Routine and immunostained sections including Melan-A and SOX10 were reviewed.

C. LYMPH NODE, LOW LEVEL 2, EXCISION:
--ONE LYMPH NODE, NEGATIVE FOR METASTATIC MELANOMA (0/1)

COMMENT: Routine and immunostained sections including Melan-A and SOX10 were reviewed.

D. LYMPH NODE, HIGH LEVEL 2B, COUNT 397, EXCISION:
--ONE LYMPH NODE, NEGATIVE FOR METASTATIC MELANOMA (0/1)

COMMENT: Routine and immunostained sections including Melan-A and SOX10 were reviewed.

Pre-op diagnosis:
Melanoma of ear, left (Multi) [C43.22]

No; You can't code I89.9 because there is no documentation supports to code. In this report scenario there is no final diagnosis so you can code from pre-op-diagnosis.
